Correcting Your Name on Form 1098-T


Overview


If your name is misspelled or has legally changed, you must update your official student record before we can issue a corrected Form 1098-T (Tuition Statement).

 

Update Your Legal Name with the Registrar


The Office of the Registrar manages all legal name changes for the university.

  1. Visit the Registrar’s Name and SSN/ITIN Change page.
  2. Select Request to Update or Change Name or SSN/ITIN.
  3. Provide the required proof of your new name (e.g., a Social Security Card, Marriage License, or Passport).

 

Request an Updated 1098-T


Updating your name with the Registrar does not automatically update your tax form. You must notify our office once the Registrar has confirmed your change.
