How to Add and Drop Dependents

Other Qualifying Life Event Changes

To de-enroll a dependent for other qualifying life event reasons not mentioned above, you must submit the Health Benefits Enrollment Form for Life Event with any supporting documentation. You have three ways to submit this form:

  1. Go to UCPath online, click Ask UCPath, click Submit An Inquiry, and then submit an inquiry and attach the form.
  2. Fax the form to UCPath at 855-982-2329.
  3. Mail the form to UCPath:
