Requesting an Affiliate ID Card


Overview


Official UC San Diego ID cards are available to campus affiliates. The current cost for a new or replacement ID card is $25.00 (subject to change).

Requesting Your First ID Card


  1. Complete the Form
    Fill out the Campus Card Request Form with your personal details.
    • Affiliate Section: Complete your name, department, start/end dates, and classification.
    • Authorization Section: Your department supervisor or HR representative must sign the bottom of the form and provide the COA payment information.
  2. Prepare Your Photo
    Your photo must meet these requirements:
    • Plain, white background with even lighting.
    • Color photo facing forward (no hats, sunglasses, or masks).
    • File format: .jpg or .png.
  3. Submit
    Email your completed form and photo to campuscards@ucsd.edu. Our office will reply with instructions on when and where to pick up your card.

 

Requesting a Replacement ID Card


If your card is lost, stolen, or damaged, you do not need to email a photo unless you wish to update it.

    1. Submit Request: Complete the online ID Card Replacement Request Form
    2. Approval: Once authorized by your department or payment is confirmed, you will receive an email notification.
    3. Pickup: Visit the Campus Cards Office during business hours to collect your card.
