Overview
Official UC San Diego ID cards are available to campus affiliates. The current cost for a new or replacement ID card is $25.00 (subject to change).
Payment Options
- Department Funding: You must provide the Chart of Accounts (COA) information on your request form. This includes a Project Number (7 digits), Task Number (1–3 digits), and Funding Source (7 digits).
- Note: A Funding Source is different from a 5-digit Fund Number.
- Self-Funding: You may pay individually using a debit or credit card via the Campus Cards Office eMarket.
Requesting Your First ID Card
- Complete the Form
Fill out the Campus Card Request Form with your personal details.
- Affiliate Section: Complete your name, department, start/end dates, and classification.
- Authorization Section: Your department supervisor or HR representative must sign the bottom of the form and provide the COA payment information.
- Prepare Your Photo
Your photo must meet these requirements:
- Plain, white background with even lighting.
- Color photo facing forward (no hats, sunglasses, or masks).
- File format: .jpg or .png.
- Submit
Email your completed form and photo to campuscards@ucsd.edu. Our office will reply with instructions on when and where to pick up your card.
Requesting a Replacement ID Card
If your card is lost, stolen, or damaged, you do not need to email a photo unless you wish to update it.

- Submit Request: Complete the online ID Card Replacement Request Form.
- Approval: Once authorized by your department or payment is confirmed, you will receive an email notification.
- Pickup: Visit the Campus Cards Office during business hours to collect your card.
