How to Access and Use the Finally Close PO Google Form.

The finally close form requires that users are signed in using their UCSD email. If users are having issues accessing the form (like the screenshot below), they should not request permission but instead follow the instructions of this KBA.

Steps to Access the Google Form

(1) Verify that you are signed in with your UCSD email

1. If you are using Google Chrome as your browser, you will see a circle with a picture or letter at the top right. You can click on that to see the details of what account you’re logged as.

2. If you’re using a different browser, visit a Google site, such as google.com, and then check if you’re signed in. You can click on Add account to sign in with a different account/credentials.

(2) If you’re not signed in, sign in with SSO

1. Go to google.com and click sign in at the top right corner

2. Enter your UCSD email and click Next.

3. You should be re-directed to sign in using SSO.

Steps to Use the Google Form

(1) Read the acknowledgements

(2) Click File and download the template linked as a copy

(3) Fill out the copy you created

(4) Add file to the form and click Submit
